That the personal details given in this form may be given to leaders who care for my child and that the details will not be given to any third party with the expection of relevant supervisory/medical/emergency services personnel as deemed necessary.

In the event of an emergency, I give permission for the team to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.

I understand that leaders will be driving my child. Leaders have a minimum of a Provisional P2 License (Green P's.)

Photos will be taken at this event for promotional purposes. If my child cannot be in photos, I have contacted Luke (luke@stpaulscanterbury.com)

I have notified Luke of any persons who is legally restricted from seeing my child.
